Patient requirements: Recovering from illness and transferring from hospital to a home setting. Patient is homebound; considerable taxing effort or unsafe to leave home. Trouble leaving home without help. Services/Requirements: Skilled nursing; nursing aide services; disease management; wound care; medication management; and physical, occupational and speech therapy. Patient requirements: Unable to tolerate an intensive rehabilitation program. Skilled rehabilitation services in a skilled nursing level facility. Services/Requirements: Physical therapy, occupational therapy, speech- language pathology one to two hours a day at least five days per week.
